This Authorization Form is provided by you, the Pet Owner or authorized representative, to Aqua Bridge End-of-Life Care for Pets (“Aqua Bridge”). This Authorization Form must be completed and signed prior to releasing your pet to us for the Aquamation process.
We want to ensure that you fully understand the information provided in this Authorization Form, so please ask any questions you may have at this point by contacting Jennifer Ryan, Co-Owner, directly at 443-537-5443.
By signing this form, you acknowledge that Aquamation is an irreversible and final process. You represent that you have full legal right and authority to authorize the Aquamation of the pet’s remains and warrant that you are the Pet Owner or authorized representative of the Owner.
Release and Indemnification: You agree to release and hold harmless Aqua Bridge (PR Pet Ventures, LLC), and its agents and employees, from any claim, liability, cost, or expense, of any kind, resulting from Aqua Bridge's reliance on or performance consistent with the directions, declarations, representations, authorizations and agreements provided by this Authorization Form. You agree that liability for any potential negligent acts (of Aqua Bridge or its agents or employees) is limited to a refund of the fees paid to PR Pet Ventures by You.
Return of Pet’s Remains: You understand that your pet’s remains must be collected within thirty (30) days following this authorization. After that time, Aqua Bridge reserves the right to respectfully dispose of the remais if no other arrangements have been made.
Disclosure Regarding Pet: You represent and warrant that your pet:
- Does not have Rabies.
- Has not been exposed to other animals known to have Rabies.
By signing this form, I acknowledge that I have read and fully understand this Authorization for Aquamation services.
